First, check to make sure the bulb and all the electrical contacts on the gear indicator panel are working. It is normal for 20 year old bulbs to burn out. You can get to it by removing the 5 screws that hold the gauge cluster surround out, popping it out, unplugging the connections. You can then access it from the back of the surround.
I just took a look at the diagrams, and it looks like if the "O/D Off" bulb is burned out or the wiring has gone bad around it that it could cause the random shifting out of overdrive that you describe. The light bulb actually works as part of the O/D control circuitry.
Like I said before though, it is normal for the car to not shift into overdrive until it has partially warmed up.
