Maybe the oil isn't reaching the top of the head? A little tick is alright, but thats an extreme one in your case.

You should pull the bolts out of the top of the case and see if any of the lifters are hitting the top of the spark plug. That could also be an issue if the wrong spark plugs were used.

full of oil. the last owner said when it started he had an oil leak and didnt drive it much cause he had another car. but needed it one day and drove it without checking oil. he got on it hard to get past someone in front of him and thats when it started knocking. got home and checked the oil and it was dry. the seals have been fixed that caused the leak but hasnt been driven much since then. it only knocks on first start up, and then when its given some gas in the upper rpms. does not knock at idle or low rpms.

ouch
don't run it anymore
either pull the oil pan or the head,
but it sounds like it might be a crank problem

person i bought my camry from ran it with no oil,
and a bearing got fused to the crank

mmm... i actually bought the car for next to nothing to get my brother by while he gets his car fixed. i was hoping it was just a lifter, rocker or something like that. if its internal i think ill just let him run it till it dies and part it out when it does.

Exhaust manifold or catalytic converter shield, catalytic converter, exhaust gasket leak worn out cam bearings [non-replaceable] or rod knock.
Make listening device using long 3/16 rod and oiler and listen to the block below freze plugs ; if noise is loudest there,
put 20w50 oil in it and see if that will help to lessen the noise.
My engine knocks like this 2 seconds after starting, so I using pep boys 20w50 all the times it is cheaper then engine

Drain the oil and look for metal shavings.
My friend however, had very quiet v-6 2.5, but drained oil revealed a lot of metal powder [one time he let the oil level to get too low].
Unplug injectors one at the time until noise wil lessen at one particular cylinder; if so, the big end rod bearing has problem in that cylinder
