2nd Generation cs pulley

the other day i was driving home when the car started to make a weird noise like a messed up bearing and dint make much of it, so i drove home and down the street from my house all the lights came on and abs, check engine, batterie, etc., well i got home and it died di i checked the charge and it seem to be fine so i went inside and next morning came i went to check the car and it turned on it struggled but it did, so i reved the engine and the crankshaft pulley flew off and it looks like it broke off from the dampener,. what would cause that? or is it just natural wear?

Using the chain wrench to hold that pulley during the crank bolt removal for a timing belt change long time ago may caused damage to the rubber, which later grew up with age. I saw the same thing on Rav-4 one week after timing belt change performed by Toyota of Cerritos.
