1994 camry speaker install questions

Im working on a 1994 camry for someone, installing front and rear speakers. Having a couple of issues thus far.

Ive searched for about 45 minutes now and havent found anything, so i figured id ask. How do i get the window crank's off on the front door panels? And how do i get the trim off around the lever?

Ive gotten to the rear speakers, those werent so bad to figure out. But the door panels are really stumping me.

If there is a walkthru or anything on this please let me know.

thanks, one more question tho..

do i have to use a speaker adapter plate for the fronts? or can i just drill some holes and mount it myself like the rears?
