Trans fluid dirty already?

Before I bought the car the transmission fluid was changed at 191K, it reads as flushed. It doesn't say that a new filter was changed though. It also sais the cooler and the TC were flushed as well and refilled with "high tech" tranny fluid. No - it wasn't Jiffy Lube

Now, at 209K miles, the oil smells burnt and is filthy. I'm changing the filter, cleaning the magnets and refilling the bitch with some new Valvoline Dex/Merc this Sunday.

Isn't 18k a little too early for new fluid?

Also, my trans doesn't shift into OD until it's warmed up for a little while (till the needle is about 20% up the scale). Is this normal?

The car will sometimes not shift into OD so that the transmission and engine will heat up a bit faster.

If the transmission fluid was really not changed until 191k miles, and it was drained and refilled, it will be dirty after only 18k because drain and refill only exchanges about 1/3 to 2/5 of the total fluid.
