AC/Heater Noise Blower Motor Problem?

I have 1999 Camry. Now, when I either turn on Fan with or without AC/Heater, I can hear big noise after it is on 2-4. I either do not hear noise or noise is very low if it is on 1.

I talked to AutoZone people. They told me that it might be caused by blower motor problem.

My questions are

1) What is root cause of this problem? Is it blower motor problem?
2) Where is blower motor? How can I replace it?

Blower motor should be below/behind the glove box in the plenum (heater core box). Blower could be bad but often it's just something which got in there and is being hit by the blades. (Sometimes leaves can get in or someone could have dropped something in one of the vents.) Usually a pain to get to.
