Check Fuel filter (possibly replace it), seafoam through the brake booster (1/3 can), and 1/3 can in the gas tank.

I'm getting about the same terrible millage.

So far i have:
Changed Fuel Filter
Ran seafoam
Changed air filter (actually switched the a true CAI) no effect on gas milage
Been using 91 octane fuel
Checked PCV
Checked Spark plugs
Cleaned TB

Next up I am going to take out fuel injectors and clean them. I'm guessing that why my gas milage is very bad.

Also I plan on replacing the O2 sensors and ETC Sensor.

That's about what I get on my 94 4 cylinder 5SFE engine, when doing mostly around town driving. Straight highway driving and I get around 30 mpg. I purchased the car new in 94. It was rated at 21 city and 28 highway.

50% city driving and 350 miles per fill up (14.5 gal) ~24 mpg combined ... And that is with light foot, i barely rev at all with this gas price.

I get 31.5 doing 75-80 with tires at 38 psi, Running 87 Octane in my '95 Camry LE
Drive 32 miles to work each way all highway.
5S-FE, & A140E trans.

use to get 20 MPG in my '93 SE V6 Running 93 Octane (Cause you can't easily find 91 octane in NH)
with the 3VZ-FE, & E53 trans. Didn't matter city or highway, always 20mpg at 32 psi.

That sounds about right.

I get ~21 in heavy (>90%) city driving, and ~32 in heavy (>90%) freeway driving

Wait... do you have O/D on or off? Because it should be on, otherwise you're not using 4th gear.

This is a very good point that many of us over look.
