Does anyone have an idea what this might be, I know it could be thousand things, but I thought mabey there is a common issue. While in park or neutral my 1999 Camry idels very smooth, but when I put it in drive with the brake on it idles a rough. ANY IDEAS???

That is normal, I brought my 99 4 Cylinder to a shop and said every thing is fine. Mine been doing that for like 3 months or more and the engine vibrates a lot.

Check RPM in gear and out. Should be something like 700ish.

If it is dropping down to the point you think the engine may stall could be the IAC valve/idle up system. This system idles the engine up to compensate for the load of the trans when in gear.
