96 Camry 1MZFE 6cyl Failed to fire up 3 times

This happened to me first, grocery store, engine turned over fine, but wouldn't start/fire up. I tried several times cranking the starter for 6 sec or less. It caught finally after about 5-6 tries. Fast forward about 6 months and the same thing happens to the missus. I wasn't there, so don't have any details. But today, she called and said it started, she backed out into the lot, and it stalled after she put it in gear, then it wouldn't start. So I drive over with the Benz and proceed to troubleshoot.
First thing I did was plug in the code reader to check for code(s). None recorded. I then tried the starter, as before, and repeated 4 - 5 times, with no result. I then floored the gas pedal and turned it over for a good 10 secs, and repeated the 'short burst' method, which finally got it started, slow rpms at first, then it picked up and ran normally.
My thinking is a fuel problem, as I recalled her mentioning 'black smoke' the first time it failed her, and a smell of gas this time. Or, maybe some intermittent electrical gremlin?
I called the dealer, actually the parts counter, and asked about a fuel filter. He said 'we don't change those, there's a filter in the tank'. I thought he was full of it, and got a price of $65.00, 'special order'. I found an aftermarket for $37.00, here in town. It's not in yet, but will be soon.

Anyone care to comment on the symptoms, possible cause(s)? Other than this, the battery is A1, plugs are all new, as is timing belt. No other related issues that I can think of. Bought it in Oct 2008, never failed to start all winter, just once in the early spring and twice this summer.

hmm its a 1mzfe and for some time my egr sensor was failing me causing my car to bog down after start and some times stall while driving. go to auto zone or something similar and pick up a can of throttle body cleaner, and maf cleaner. its cheap, quick and may help... keep us posted with info.

I should have added that I also recently did spray out the throttle body with TB cleaner, and sprayed in MAF cleaner too, in an attempt to cure what I consider a 'rough' idle...not erratic or too high, just not smooth. It makes the steering wheel vibrate slightly. That was the main reason for putting in $65.00 worth of new plugs too...none of which smoothed out the idle. The first time the car wouldn't start, as described, it was before the plugs and TB/MAF cleaning, the last two times, after.
Anyone have any other thoughts?
