Radiator Fan Meltdown

The drivers side radiator fan on my 1995 Camry melted out of it's housing and melted the fan itself.
What would cause this ?

Is it just a bad fan motor or is there an underlying cause that will ruin the new one that I'm going to put in ?

Could there be a faulty sensor that made this motor run wild and over heat ?

Help Appreciated.

May be over-current? If so it's a surprise that a fuse didn't catch this. A new assembly should be fine, however will set you back a few bucks. Not sure if you want to go the EBay route, but it's worked for me too.

Yes, I have pics.
I just cut the thing out of the shroud and took the pics.
I'm headed home and I will try to post them.
I have never posted pics here since I'm new to this forum, but I'll figure it out.
Thanks for the quick response gentlemen.
Stand by.

Here is the Pic of the melted fan.

Used fan and shroud from junkyard, $50
New aftermarket radiator, $135
New rubber radiator feet, $32
3 Hours casual labor.
Technical difficulty of fix, on scale of 1-10, ...about 3.
Seems to run fine.
I think the problem was the motor not the relays or sensors.
Amazing how hot this thing got.
It melted the shroud/support (black plastic glob) as well as the fan.
Thanks for your help.

PS This meltdown did blow a 30Amp Fusible Link
The Fusible link is Marked RDI something in the fuse box.
The fusible link for the other fan is marked CDN or something and stands for Condenser fan.
