Sounds like either the Throttle Position Sensor is having problems, or the Idle Air Control valve is really, really messed up (not just dirty).
First, I'd inspect the TPS (see
http://www.camrystuff.com , Gen3 manuals, 1MZ-FE Engine (V6) section, page 264). Especially check between E2 and IDL -- should be a short with the throttle fully closed (the idle sense switch is "on"), and an open with the throttle is open (the idle sense switch is "off").
If the idle switch is OK, then things really point at the IAC -- the insanely low idle, hot or cold, is kind of a classic "IAC ain't opening" symptom (but I always like to make sure the computer knows the rig is idling, thus the TPS inspection).
IAC is tougher to inspect except by inference (i.e., if you disconnect the IAC, and your idle doesn't change, something is rotten). Gen 3 FSM (
