Recommended service is every 60K miles. If the belt has never been changed before, you're living on borrowed time.

The 1mz-fe and 5s-fe are non-interference engines. Nothing other will happen when it breaks.... other than you being stranded. But you are living on borrowed time. Get your belt and water pump replaced. There should be a DIY in the sticky if you feel mechanically inclined to change it yourself.

Yeah, do a complete job that includes oil seals, new pulleys, water pump, and all belts. Parts should be about $200 with online dealer.

Good time to inspect hoses and engine mounts among other usual suspects during a major maintenance interval.

^ Yes, but I'd only go with OEM parts.

T-Belt, Water Pump, both Cam seals, Crank seal, Oil pump gasket, Tensioner.
