My LTFTs deviate about 3% bank to bank on my 2000 1MZ with air-fuel ratio sensors. The front bank sees 6.25% and the rear bank sits at around 9.25%. The only thing I can think of is the rear bank's rail has less pressure because it's a returnless system. You have a return system, so your fuel rails should have equal pressure. My STFTs are 0% because AFRs give the ECU the exact air-fuel ratio and no longer need to switch between rich and lean running.
The 1MZ has bank to bank fuel control, so the ECU can run independent settings for each bank. This helps idle and overall performance. I'm not sure what's going on in your case with your erratic engine performance.
I have ported heads, am 3000ft above sea level, and run 91 octane E10 gas. Coolant temperature sits at about 180F.
