Coolant and fans are not moving

I noticed that my coolant never goes up and down the reservoir. I've also never seen my radiator fans operate. My upper radiator hose is also hot after I've driven my car. My car does not overheat, and the temp gauge seem to be normal (a little less than the halfway mark).

1) Is there a cause for concern ?
2) Under what conditions would my fans operate ?
3) Under what conditions should the coolant in my reservoir move.

My car is a 1998 4-cylinder Camry.
