Is this EGR Vacuum line diagram correct?

I'm trying to reconnect my EGR so I can pass emissions. I'm getting the P0401 code "Insufficient EGR Flow" and I'm pretty sure my vacuum lines aren't hooked up correctly.

In the second image, I highlighted all the vacuum lines that seem to affect the EGR. If I understand it correctly, the Vacuum Tank has nothing to do with the EGR.'

I hooked up the EGR vacuum lines according to the ones I highlighted, but it seems it hasn't fixed my EGR.

A little back story... my EGR system was removed back when my supercharger was installed because I didn't have the right EGR pipe to fit the supercharger and we didn't feel like dealing with it.

Any ideas?

I recently did my valve cover gaskets so I needed to keep track of all of the hoses in what I call the Rats Nest.

I'm not very good with diagrams, and as they say, a picture is worth a thousand words.....

The lines that go to vacuum tank are kinda weird. One of them comes off the back port on the plenum, and the other one comes from the Rats Nest. You can see it more clearly in this pic.

Hope this helps.
