I have an 03 Camry that has a clicking clacking engine noise when started, is sounds like a lifter or a valve; however after letting it idle for about 20mins, the noise disappears and it runs fine. So far i've done.
1. Flush Engine
2. Change oil and even used a very thick oil 20w 50.
Here is a video clip of what it sounds like with the noise and without the noise towards the end of the video.
Does not sound like a lifter, sounds like a bad/spun piston rod bearing. Did you run low/out of oil? Run an extended OCI? All sludged up? Running thicker will only make it worse. An Engine flush can also cause problems. Im afraid there's nothing stuck, its broke. A teardown is a major risk in these years, google stripped headbolts. Head to a mechanic asap. In the meantime throw in some M1 High mileage 5w-30 and pray.

Thanks, I might just have to head to the mechanic's shop. However if it is a bad piston rod bearing, what makes it run fine after sitting idle for a while. Also you think the oil might cause more damage?
Nah, not at this point. Most of the time rod bearings go due to oil starvation or dirt/sludge. A thicker oil will take longer to flow when its cold so there is improper lubrication causing wear at startup. Add a thick oil to a sludged up engine, ouch. Your spec is 5w-30. I've had luck quieting lifters over the years with Marvel mystery oil, but i think your beyond that. May help thin your oil some. Had great luck with the M1 HM 5w-30 its both a cleaner and has great wear adds. Just think you need to have a mechanic look at it and give you a quote.

I'll give that a shot and see if I do get lucky indeed. It just baffles me that it sounds and runs fine after idling for a few minutes. Thanks for you suggestions.
Because your getting better oil pressure after a while. I have an 02 Camry 4 cyl like yours (186k) Cant hear the thing idling its so quiet. Was a ticker at first. Took 3 OCI's with the M1 HM to get the crap out from the previous owners neglect. Change the oil with what I recommend above, I use the Fram Xtended guard filter XG4386. That filter can catch a ton of gunk, not overly restrictive. Its a "gold can", box wrapped in plastic. Walmart should have that too $8.97. With that setup your good for 5-6k miles. Lemme kno how it goes.

that's an awful sound. good luck with it, but i'd prepare for the worst--that actually does sound top end-ish and not something that'll go away using different oil.

It sure is an awful sound, using a mechanics stetoscope the sound appears to be coming from the top part of the engine from the first cylinder on the right(facing the engine). I'll take it to go see a mechanic this weekend hopefully. Thanks.
The problem was with the Camshaft and the noise went away and engine smothed out once the camshaft was replaced........ I'm glad it didn't require an engine replacement.
