I am experiencing the exact same issue on my 99. I just replaced the door glass run (weather-strip around the window) on the driver's door, after experiencing noise coming from the window. The first weather-strip was deformed in the upper corner, allowing wind noise to come in. When I changed the strip last night, I pulled the door apart, and did remove the window before installing the weather-strip which I know is correctly sitting in the channels in the door. Essentially what happens is the window does not sit in the channel as it goes up, and pops out as it gets to the top of the window next to the B-pillar. I have noticed that if I roll up the window up to about an inch from the top, I can pull the window towards the B-pillar, which better seats it in the channel, and the window rolls up correctly. I have tried to "adjust" the window motor, and window glass, but it doesn't seem there is any play to correct this behavior. Any ideas?
