Rack & Pinion Problem?

Now Here's the problem of my 94 camry.
First when i take a turn to right, when the steering wheel pull back to the center, the alignment will pull to the right.

Second, After the right turn, if i take a left turn, the alignment will pull back to center again.

Third, CONTINUOSLY, if i take a left turn again, the alignment will pull to left again.

So can anyone tell me what's the problem?

I have asked my friend, they said myabe the brackets which tighten the rack and pinion are lossen? IS that true?

How do i check whether the R&P is broken?

Another problem is, when my car totally stopped, i try to turn my steering to left and right. Then there was a strange sound "Thud...THUD..." is that R&P problem too?
