Every 5k or every other oil change if you are doing you oil changes every 3k. It's a good idea to get lifetime balancing and alignments and take advantage of it.
Depends on where you do it. it can be really cheap. Like when you buy a set a lot of places will do it free forever.
Rotate often(I do it every oil change 5-7500mi) so you can keep the worn tires on the front. It is very dangerous to drive ANY car with the worn tires on the back regardless of the drivetrain it utilizes.

Its all in the Haynes Manual..Why won't you people go buy one?
Rotating Tires means you're just rotating the wheels from one position to another. No switching of tires on rims or any of that crazy nonsense.
Also, if your tires cup or scallop dude your either:
under-inflating
over-inflating
or your camber is off
Tires DO NOT cup or scallop or wear unevenly under NORMAL driving circumstances.
**Camry people with non-directional tire follow the front to rear rear left to front right and rear right to front left
***If you replaced your tires with directional tires you should go front to rear and rear to front - NO switching sides~!! (Check your tires side-wall where the info is...i.e. where the name is all directional tires are marked with an arrow indicating direction of the tread) VERY IMPORTANT THAT IT IS GOING THE RIGHT WAY~!! Be sure to check that the shop has put the tires on right before leaving~!!!!!!!!
****Be sure to check for nails and what not, if you see a nail DO NOT REMOVE IT(unless its tiny) - Take it to a shop and have them use a patch with a plug~!!! No shoddy home repairs with only a plug~!!
*****Punctures in the side-wall cannot be repaired~!!!

I do it every oil change (5k), not that it is necessary that often (even though I drive them pretty hard), its just easy to do when you already have the car up on stands. I was told to do the X pattern, unless they are staggered sizes or directional.

Also, most places will only rotate your tires if you got service done from them or bought tires or rims.
To ensure that it is done the right way take 30mins - 1 hour and do it yourself on a Saturday.
You do not need jack stands though 4 would make it very easy.
1) Take out your spare
2) Start at any corner and remove that tire replace it with the spare and 3 lug it..all 5 if you're anal and afraid you'll scratch the thread
3) Move to the correct position you should be rotating to.
4) Rinse and repeat
Hypothetical Rotating Situation for Non-Directional Tires Front Wheel Drive(Thats a Camry) Vehicle:
1) Start with rear left
2) Jack remove tire and replace with spare
3) Move to front right repeat and replace with tire from rear left
4) Move to rear right repeat and replace with tire from front right
5) Move to front left repeat and replace with tire from rear right
6) Move to rear left remove spare put on tire from front left and you're golden
Nice workout and your tires are rotated
Good time to clean off the insides that are most likely caked with road crap and brake dust

In the manual, it says that you should rotate your tires by taking the front left tire and switching it with the rear right tire. and the front right tire with the rear left tire. In order for you to do that(and keep proper rotation), your supposed to take the wheel off the rim and switch it with the other one. (Formng a X pattern)
Im just stating what the manual says. I dont even rotate tires here in NYC anymore cuz potholes blow them/shred them out before I have a chance to.
Ill let you guys kick that around a lil bit.

Thats the old school way of doing it. You are supposed to do it from front to back. By cross rotating them like that you are now making the tires roll in the opposite direction it was going before. By cross rotating them you can cause a tire seperation and/or bad tire wear.

Thats why I said earlier that if we were to do it the way it says in the manual, you'd have to strip all the tires and switch them to keep them rolling in the right direction.
