Sand or use a wire wheel attached to a drill or angle grinder. Grinder would work better though.
Clean it off before you paint it. Acetone works good for cleaning metal.
If your going to paint it, use high temp engine paint. DupliColor makes a engine paint that holds up to 2000 degrees. I don't know how well it'll hold up though, but the worst it'll do is burn and flake off.
Your other option is to have it bead blasted or sand blasted and ceramic coated inside and out. Downside is that having this done is going to cost you more then what you paid for that manifold/header.

Sound's like the better option to me. And is all the more reason why getting a cheap deal on the header best, saving money for the resto.
