Unhappy Shudder in reverse

Have a 1993 Toyota ext. cab truck with 4 cyl. auto and 220,000 Km. I am getting a shudder when the truck is put into reverse. The shudder or vibration can be felt in the steering wheel and the front fenders. I get this only when the truck is under a load, for example, backing up a hill. I can mimic the condition by putting the tranny in reverse, holding the brake on and reving the engine slightly. I suspect a problem with a transmission band (adjustment needed?) or a clutch? Wondering if anybody has had a similar problem or knows what the problem is. Help is greatly appreciated.

Rear brakes are adjusted up. Not sure how this could cause my problem. Could you explain?

if the brakes stick on they will slip inside the drum.
