1st Gear Pops Out

My Toyota truck 5spd tranny just started popping out of 1st gear on take off but seems to be getting worse. Is this an adjustment/fluid kinda thing or something more serious???

Does the truck have a lot of miles on it? The 1st gear synchros is probably worn-out.
