Battery wiring and grounding questions

I've got a '94 Geo Prizm 1.6 MT and have noticed the engine won't start from time to time, but adjusting the battery wiring and it gets power. Looking at the battery cables, they've seen better days. So I decided to replace them. This leads in to my 2 questions:
1. On the negative (-ve) terminal, the cable on my car has two grounding wires that hang off the cable. One ground wire runs from the -ve post to the body. There is a second grounding wire (two parts) at the other end of the main cable. It too connects to a different point on the body. The two parts are joined by a connector with a plastic shroud. The main cable runs from the -ve battery post and attaches to the metal support bracket for the right side (driver side) engine mount. Is this correct?
2. I replaced the -ve post cable with a new length of 4 gauge wire. What do I need to do to replace the positive (+ve) cable, as I see it runs into the main harness?
