Jerks in what way - is it in the transaxle or the engine?

If this only does from a cold start or after the car has sit in one spot - I would suspect an issue with the slave cylinder or master clutch cylinder. If those are leaking, have worn seals, or have air in the line - it will act funny until it gets some use.

The same reasoning will apply to the clutch disc itself. Could be contaminated, heavily glazed over, or just plain worn out. Get a lot of clutch slipping, jerky engagement until you heat up the disc to the point where it starts to catch.

A sticking clutch release bearing and/or sticking clutch fork could also cause you some of these headaches.

Try changing the clutch fluid first - takes plain old DOT 3 brake fluid. If the fluid in the reservoir is heavily discolored, you might have a significant amount of moisture contamination. That moisture will eventually lead to corrosion that in turn, will eat the seals up like crazy. That point, you have no alternative other than replace either the slave or master clutch cylinder - possibly both. Changing the fluid will also give you a chance to purge any air that may have gotten trapped in the lines.
