Need more information - exactly what do you mean by "unhealthy"?
What is the model year and mileage of the car?
What was the maintenance history of the car?
Was the car hard to start, slow to turn over, etc.?
Give a more detailed description of the rumbling noise?
Fluids all topped off and in good condition?
Could be anything number of things - from a loose heatshield, to bad gas, to generally cold weather, poor maintenance - excessively viscous motor oil, worn plugs, ignition issue, weak battery, etc.
You can't tell how much it costs without doing some diagnostic work first - or you could just take it into a garage and pay about $85 for them to do the diagnostic for you.
If you stayed on top of the maintenance, all fluids checked out, and the car hasn't done it since then - probably nothing wrong with the engine at all.
