It's making a knocking/rubbing sound. It gets pretty loud. The mechanic said it's the tentioner. It still works but it's just loud. Is it safe to drive; how do I know when to replace it, does it harm anything else when driving?

Apparently it is still functioning enough, but if it completely goes out on you your car will not be drivable. No water pump, powersteering, alternator.
