From what I understand, the 2003 model year transmissions had some parts that were prone to wear (like synchronizer rings or something similar). The parts were upgraded in 2004. The 5-speed in my '07 is smooth like butter. It's really a pleasure to drive. Most of my driving is in "econ mode", and the engine is all but inaudible and all you can hear from the shifter is a muted snick-snick as it goes through the gears.
The 1ZZ-FE engines from '03 until about '06 or so had serpentine belt tensioners that liked to make noise. Toyota has a TSB about it, and the unit has been re-designed. They're not all that expensive to procure if the one you do buy makes some noise, and replacement is fairly easy.
