'96 Avalon XLS Gas Tank Door

Took the problem to the local Goodyear dealer where a Service Tech tried the credit card bit...didn't work. He unopened the Gas Tank Door Lever housing to the left of the driver's seat. This exposed a red colored cylinder that incorporates the end of the cable to the gas tank door. The red cylinder had come out of a slot and needed to be snapped back in. As soon as it was and the housing replaced, the lever worked and the door opened.

THERE IS NO LEVER IN THE TRUNK OF PRE-2002 TOYOTAS WHICH WILL OPEN THE GAS TANK DOOR!

The same thing happens on my '99 Avalon every few months. The head of the cable somehow falls out of its slot and I need to pull off the lever assembly (black thing with the trunk/gas levers on it and two phillips screws) and pop the cable head back in. I have no idea how the head falls out, there is always a little tension on the cable to keep it from doing so.
