Loud screech at start up

Maybe 1 out of every 40 times I turn the key to start my 99 avalon (191k miles or so) it makes a loud screeching noise, not really high pitched though. If I take the key out and try again it will start 99% of the time on the second try. I'm guessing it has something to do with a starter gear being worn, but I don't really know the names of many car parts. Anyone know what I need to replace and if it's difficult to do (or costly if I can't do it)?

Is it a grinding or screeching sound? Do you mean the starter won't turn the engine over the first try?

A sticking starter drive or bendix, will cause a grinding sound and it may not engage properly to turn the engine over. If that is the case, with so many miles on the car, a starter replacement would be the best way to go. However, the ring gear on the flex plate should be inspected for damage, if damaged, the transmission will need to be removed to replace the flex plate.

correct, and the noise is like when you pull a cord on a mower or dirt bike and it doesn't start, except a lot louder and for as long as I hold the key.

I would definitely remove the starter and check the drive, and inspect the flex-plate ring gear for damage also.
