2001 Corolla problem

I got this message from my buddy this morning. And I couldn't think more than transmission. See if you guys could see something else. The cas is manual transmission.

"sometimes when I step on the gas the engine balks, then takes off. Sometimes when I'm accelerating hard (say, in 3rd gear getting on the freeway), it kind of "pulses", not jerky, but smoothly. Hard to describe: it's like the rate of acceleration changes. Like it goes hard for a second, then less hard, then hard again. Like a sine wave in Calculus..."

As for the "balks" i donno as for the surging sine wave comment the engine doesn't have a linear power curve it will have more power in certain RPM ranges so that could just be it, if he's not sure id take it in and have it looked at
