5sfe is every 30,000mi (per the dealer). Haynes says 60,000mi. I'd imagine your motor is probably the same. Pick a number. The PCV functions to let the crank breathe a bit, you let it get clogged up and you may run into my motor situation.
My mechanic at work said to check if it's working take it out and rattle it. If it appears to move freely then it's still good. If you get no rattle then it needs replacing. Anyone here want to verify this diagnostic tidbit?

Usually located on the valve cover. Its a one way check valve, so to test it, blow into it. If you can blow in one way, but not the other, then its okay. If you can blow through it both way, its fucked. Its most likely a little black "L"-shaped plastic piece about the size of a lighter. Its very easy to replace, just pull it out, and plug a new one back in. It will usually have a hose connected to it, which recirculates the blow-by gases back into the intake tract.

What Ratko says about checking it is right, but it doesn't exactly look like that on a 1zz-fe.
Since this is the Corolla forum I will assume you have an '03 Corolla. (BTW all Corollas have the same engine)
The PVC valve is located on the right side of the valve cover. First take off the stupid god forsaken piece of plastic that says "16V VVT-i" on it. There are two real bolts towards the front and two piece of shit plastic connectors towards the back that I ended up braking the first time mine came off. The PCV valve is screwed into the valve cover on the right side (driver's side) It has a black hose connected to it. Just take off the hose and use an adjustable wrench to screw out the PCV valve, then do the blowing thing Ratko said. It doesn't hurt to check it but it will be fine. You need to replace these once maybe twice in the whole life of the car.
