Actually it would be your throttle position sensor, or more likely the idle speed control sensor located on the t/b under the throttle plate. Take the entire sensor off, and leave the harness
hanging and see what difference that makes at startup.
I had a similar problem, removed it, now the car starts at about only 1400 RPM and drops back 1000 (1000 keeps the body from shaking for me).
You can test the contacts on the ISC sensor to see what the resistance is at. For a '91 5sfe it is like 19.2~23.0. somewhere in that range.
Try cleaning the throttle body throughly as well. You may have a sticking plate.
