Overheating problem

Hi,Just recently my car overheated due to a total loss of coolant!
I filled it up again and so far so good.I have noticed that when ever my car gets up to normal temp the water in the header tank is boiling furiously.Tha car warms up as normal and I have felt all the hoses and are hot so I don't think that the thermostat is faulty.The coolant level seems to drop due to this, and I have to keep topping it up,could the radiator cap be faulty?
P.S The car is a 1990 2.0 GT 3SGE.

Thanks for reply.Is there any other way of telling it is the head gasket, as the oil is not at all sludgy, it is clean, so no water there, and it does not blow any steam out the exhaust other than on startup.
How can I tell?Thanks.

Try new rad. That's what mine needed. Lose coolant due to coolant boiling. Overheat only when I drive it hard.
