Vaccuum Switching valve could be bad. happens alot on the ES250. but check all of your vaccum lines.
aonther option is a gunked throttle body /intake. get some intake cleaner! LOL.
and shut up about your mileage! i WISH i got 22mpg. i average less than 15 on the highway, and all the stuff you listed is new on my car too.
