2001 Toyota Sienna Rear Passenger Door will not open

The passenger side door of my 2001 Toyota Siena (manual door with power locks) will not open. I can only open the door taking off the interior panel and directly accessing the mechanism inside the door (see the below link for a picture of my door). The door only opens if I push on the black mechanism in the direction of the arrow. What parts do I replace?
See the picture at http://www.substantis.com/gallery/album269

To expose the problem, I had to remove the door panel (taking out the cup holder, removing the screw hidden in the cup holder, reinstalling the cup holder and pushing down and pulling out on the door panel as suggested elsewhere in this forum).

I have not looked into how to fix this yet but I have the same problem on the drivers side sliding door.

Check (embarassing, but yes, I've been there too!) the little childproof toggle on the edge of the open door. If you brush against it while, say, loading, and it gets put to the 'kiddie-safe' setting, the inside control won't work.

And then, if say, an enthusiastic teenager has yanked the handle off the outside, you have to (IIRC) fish for the cable to get that door open...

I've considered (for adults-only, and no grandkids scenario) putting a big sheet-metal screw in next to that toggle, just to keep it from getting pushed over. Removable if ever sold to parents.
