Check (embarassing, but yes, I've been there too!) the little childproof toggle on the edge of the open door. If you brush against it while, say, loading, and it gets put to the 'kiddie-safe' setting, the inside control won't work.
And then, if say, an enthusiastic teenager has yanked the handle off the outside, you have to (IIRC) fish for the cable to get that door open...
I've considered (for adults-only, and no grandkids scenario) putting a big sheet-metal screw in next to that toggle, just to keep it from getting pushed over. Removable if ever sold to parents.
