check engine, vsc, trac off

2009 sienna, check engine light, vsc, and the trac off light came on. took gas cap off and replaced it and nothing happened. check the codes and it came up with p0138, p2195. and p2195 pd which is the o2 sensor. I cleared the codes and check engine light, vsc, and trac off light came back on today. did the other work yesterday. today 3/27/11. haven't checked the codes today but more than likely will be the same. so what suggestions does anybody have. car has 48,500 miles on it.
Thanks

Had you been driving the van a while when it happened? Did the check engine light stay on steady, or did it blink? If it blinked it could be your catalytic convertor going out. We have an 2009 Sienna LE and on a long road trip we had the exact same thing happen. We managed to limp it to a dealer who pinpointed the problem as the catalytic converter. Dealers aren't too wild about replacing a $2k catalytic converter, so stay on top of them because it is covered under your extended warranty.

Also, I wouldn't sweat the vsc and trac off lights yet. My wife hit something on the highway and busted the vapor canister on the bottom of the van. The CEL and VSC / trac off lights all came on. As soon as I replaced the vapor canister all 3 lights were gone. Still not sure why the VSC and trac off lights came on the first place.
