I have the same car, i think if i were having brake fade problems i'd consider buying a ventilated (drilled) rotors if they have them available. I work on ford crown vic police cars and when their rotors wear out, we replace them with special drilled rotors- and those brakes NEVER fade.
Also i'd consider a switch to synthetic brake fluid since the Dot 5 is probably not meeting your driving requirements. The downside is that you have to take the brake system apart and clean it to switch between OEM brake fluid and synthetic.
