There are probably lots of possibilities. I had similar problems a few years ago on a Mazda. Multiple trips to the dealer did not help. After I got rid of it, someone suggested that loose manifold bolts could have been at fault. It wouldn't hurt to torque the intake and exhaust manifold bolts. Good luck. Let us know what you find.
