95 toyota pick up running rich

i am having truble with my 95 pick up with 22r e
my check engien light says i am running lean code 25 but my spark plugs and my fuel milage say i am running rich and when i am driving on the freeway my arf is 9.11 extremly rich when i come to idle it runs 15.9 or so the truck got a new motor about 50000 miles ago alonr with header water pump radiotor and k&n filter also a new o2 about 2 weeks ago please help i dont know what to do

ive got the same truck with mods and have the same problems. but mine only does it when it wants to. sometimes it will run great for a few weeks then start running bad again for a few weeks
