Are you sure the clutch is ok? Have you bled the system?

I had a similar problem, but here's my fix:

Remove the shifter, then remove the plastic bushing at the bottom of the
shift rod. Replace the shifter, and try it then. For some reason, with age,
the bushing prevents shifting into 5th and reverse. Can't explain it, just
know it works. Good luck.

P.S. I just helped someone with the same problem. To remove the shifter,
just unscrew the rubber boot, pull it up out of the way, and then undo the
retainer by pushing down and twisting left. With the retainer unclipped,
the shift rod should pull right up.

Re: toyota truck manual transmission

i found the answer. For a 1989 toyota 4wd pickup.5 speed. Remove the
rubber boots, push down the retaining clip and turn counterclockwise
and the shifter will pull out. there are 2 bushings that need to be
replaced. one on the end of the shifter,(looks like a little cup) . I
cut the old one off, The new one can be tapped on with a hammer after
being lubed. The other one sits just below the dowels that the cone
shaped part of the shifter(with the slots in it) rides on. clean the
shifter and apply some bearing grease then reassemble. These were in
stock at the toyota dealer for $18.18 . The truck now shifts like new.
I hope this helps someone else.
