with a lot of swearing and cramped fingers usually :-p
-2 ways to get at the nut on back (toward the rear of the car): either put a wrench on it from the engine bay (hard to reach/turn) or use about 3 feet of extentions on a ratchet and go along the length of the transmission (hard to see)
-the head of the bolt also has 2 ways: remove battery and unplug stuff going to the fusebox, insert arm about there and reach back with the wrench. I use this one a usually cause I have long arms. the other way is doing the same and threading extentions in till you end up with the ratchet in a reachable place (under and in front of the alternator)
this and the fuel filter are the 2 parts toyota really botched on as far as ease of repair/removal.
