When removing the passenger seat cover it was stuck in the reclining gears. there is small tearing on both sides.
Nothing that would run and totally not visible when installed. SEE PICTURES BELOW

Does the iPod adaptor show the track name on the toyota radio? Is it compatible with the 2ng gen iTouch?

Yes, on my 6 disk cd I can see the track name / band - although it is truncated after 10-12 characters. Not sure about I-touch

Does the weather tech fit an access cab?
If so what would shipping be to 11701?

I don't believe they would not fit an access cab. You can check with Weathertech to see if there is a difference in what they offer today.
