'03 Highlander problem:

My '03 Highlander with 170k miles on it intermittently seems to pop out of and rapidly back into gear, accompanied by appropriate thumps and jerks, when reversing uphill. This symptom can be induced by holding the car stationary with the brake and giving it a little gas while in reverse.

Have you checked the fluid level of the trans? If high mileage, has the trans filter ever been replaced?

Transmission fluid level is good and the color is still pinkish, but hasn't ever been changed or flushed, that I know of.

Then I would suggest draining the trans, dropping the pan, and changing the filter. And when it's all back together, add a 10 oz bottle of LubeGard Red to the regular T-IV fluid. LubeGard is a friction modifier that can free up sticky solenoids and valves, and may solve your problem. This is the cheapest way to start trying to fix your problem.
