I just installed Bilestein HD's and I set the washer's up like my old shocks. Here's how I did it: Bottom one is correct, top one should be in the middle and in the same orientation. Middle one should be on top and flipped over from how it is shown in your picture.
On the truck from the bottom going up it will be: washer - bushing - washer - mount - bushing - washer - nylock nut.
Again, I set this up exactly like my old shocks were, whether it's right or not is another story... As you've probably found the Bilstein directions are pretty much nonexistent. Be careful not to torque that top nut down too tight as well. My FSM had a torque spec for that nut but when I torqued the new Bilstein nut to that spec it crunched the bushings big time. I backed it off a bit and called Bilstein to get a better idea. They told me tighten the top nut until you begin to compress the bushings and then 5 more turns thereafter.
