The fan in my 2011 Matrix is only blowing on the highest setting (4), heat and air is ok. Is this a fuse issue?

If it were the fuse fan would not work at all. More likely blower motor resistor.

I had this happen on my 2007 Corolla when I bought it used. I ended up finding the part behind the glove box & I took it off. I think it was only a single 10mm bolt. I then found that the solder was broken & after I fixed it it has worked perfect ever since. :D
Blower motor resistor 99%
Do you replace your cabin air filter? If it gets clogged enough it can reduce air flow over the resistor and it burns out. Not that it can't fail for other reasons.
If you are only getting high speed, then your blower motor resistor is blown. High speed gets direct battery voltage, all ther speeds as are feed through the resistor. The resistor is located behind the glovbox. If you look at the heater box you should see a connector that plugs directly into the blower motor resistor.
This is what it looks like on a Corolla. Not sure if it's the same on a Matrix.

Heater fixed

Sorry I'm late posting. I replaced the blower motor resistor, $ 30 and 15 minutes, local shop wanted $125. It was in the same location as shown in the Corolla pic. Thanks to everyone for your help. I have to keep Mom warm, she will be 86 years young in March!
