yes you have a leak. Take it to the dealer or get a coolant leak detection kit.

you may want to get that checked to be on the safe side. over time.....a little bit of coolant will evaporate.....but having to add some every three months seems a bit excessive to me. i've had to add about a cup to mine in the little over two years that ive had my prerunner.
