1986 22R Timing chain keeps jumping a notch

I have an 86 22r that was rebuilt 20,000 miles ago. Since then, the timing belt has jumped on three seperate occasions. As I understand it, tension on the belt is maintained by oil pressure pushing a tensioner against the belt.

Any ideas why this would keep happening? I have heard the Toyota oil pump only puts out 10 psi? Could this be an oil pump problem? Would a higher output oil pump be of any help? I love this truck- bought it new and have put 230,000 miles on it, but these are expensive repairs as I don't do the work myself so unless I can figure out why this might be happening I feel like I need to dump it.

I'd wonder if it was installed properly, did the same person/shop do it each time?

The only other thing I can think of is the oil passage to the hydraulic tensioner is clogged, so the tensioner isn't working properly or at all.

I've had a bunch of 20R and 22R engines apart. Not much besides those parts can cause this. The guides all need to be replaced in those engines when the chain is done. The tensioner as well. If they are all functional, there really isn't much else to go wrong. You do need good oil pressure but the chain jumping is really odd.

Possible that the chain is stretched or just a bad chain I guess.
