Once I got a GENUINE TOYOTA O2 sensor the issue was solved. The Denso sensor that I bought from eBay was crap. O2 sensors are one part that I will only buy from a Toyota dealer. Currently at 104K miles.

I just replaced the right rear door handle and the door lock actuator this morning. Total cost in parts was $37.00!
