How often should the fuel filter be replaced in a 2004 4WD Runner? Knowing it is in the tank, is this something a mere mortal can replace or is it suggested that the Toyota gurus replace?

^And with that being said, they are good for at least 100k. Good chance that the owner's manual doesn't recommend replacement at all.
Fuel filters should be replaced every 5 years or so... not so much because they are getting dirty or clogged, but because they can disintegrate and plug all the fuel injector screens if neglected.
We have injectors come in the injector shop now and then with parts of the fuel filter caught in them.

Toyota claims the fuel filters are for life and never need to be changed unless they are damaged. They are kind of out there hanging on the frame, so if you hit something you might damage it, otherwise you should never have to mess with it. As stated before, most owners manuals don't even mention it as ever needing service.
